Sole executor for the immutable handler and target knowledge in MutationPlan.

Interceptor outputs form ordered substeps under one pinned read boundary. Within a substep, payload handlers first produce one immutable intermediate state and model handlers then consume that complete state. Handlers within either phase never observe partial sibling results; only the composed final state becomes visible to later substeps. This class owns handler invocation, result validation, ordering and staged Graph changes for live handling, retry, rebase and reconstruction.
